How the ClearMind 401(k) Scorecard works

Every 401(k) plan files an annual report with the U.S. Department of Labor, called a Form 5500. Small plans file the shorter Form 5500-SF, and the DOL publishes the answers as public data. Find your plan by name or EIN, confirm it’s yours, and we score the latest filing the DOL has published. You see your overall score and your plan’s story right away. The score is built only from public filing data. It isn’t an audit, a compliance determination, or investment advice. A filing can’t show fund fees or how the lineup was chosen, so Austin Wolfe and Joe Anderson cover those with you in a 30-minute walkthrough.

What the Scorecard checksShow the five areas
  • Bond and compliance answers (25%). Whether the fidelity bond meets the legal minimum, and the yes or no compliance questions, like late employee deposits.
  • Audit threshold (20%). How close the plan is to 100 participants with balances, where a yearly independent audit can be required.
  • Automatic features (15%). Auto-enrollment, a default investment, and how much the people who save put in.
  • Corrective distributions (20%). Money sent back out of the plan, often after a failed IRS fairness test.
  • Who signed the filing (20%). Whether the company runs the plan itself or names an outside administrator.

Plans that file the full Form 5500 (usually 100 or more participants) aren’t scored automatically yet. Pick “None of these” and we’ll build yours by hand.
